April 15, 2021 First Dose Moderna Sign Up April 22, 2021 First Dose Moderna Sign Up Name, address, and phone number are required for an appointment. The clinic is at the Ionia County Road Department. It is drive-through. Individuals can arrive anytime during the one-hour slot that they sign up for. They should bring ID if they have ID. While we give preference to those who live/work in Ionia County, this clinic is open to anyone who is eligible (18 and up; not currently COVID-positive or under quarantine). Individuals will stay in their car throughout the process. They will be checked in while in line, go through a pre-vaccination questionnaire with a HD staff member, get vaccinated, and then park and wait for 15 minutes before being released. The 15 minute wait is required and is to check for immediate allergic reaction. The entire process usually takes about 30-45 minutes.
